How to Apply for Eligibility

Visit www.cosmeticdentistrygrants.org and apply for consideration of a cosmetic dentistry grant. Schedule and complete a complimentary assessment with a participating dental practitioner. (At all times, all basic or general dentistry costs, if any, will be incurred by applicant). All applicants must have completed an application form. No incomplete or forged applications will be accepted.

Applicants must apply for CDG funding and be accepted into the program prior to the commencement of any cosmetic dentistry treatment.

Ineligibility

Any applicant that is actively and currently engaged in any phase of reconstructive dentistry e.g.:, bone augmentation, placement of implants and/or the prosthetic (finishing) phase of implants (placement of crowns, bridges and or implant supported dentures) may not qualify for consideration of a Cosmetic Dentistry Grant.

Applicants may not apply for CDG funding for the purpose of settling prior payments made, or existing payments due, for any cosmetic dentistry treatment that has already been completed or is about to be completed.

Complimentary Assessment

Complimentary assessments are provided to applicants when the applicant does not have insurance, and when the assessment date is booked within 2 weeks of the applicant being accepted into the CDG program. Upon booking an assessment, the applicant must provide the dental office with 48 hours advance notice to re-book. If the applicant fails to comply with these provisions for receiving a free oral health assessment, the dentist may, at his or her sole discretion, charge an assessment fee.

Selection of Grant Recipients

Qualified applicants will be notified by email and regular mail of their acceptance and the terms and conditions for redeeming their grant award.

Awards

Cosmetic Dentistry Grants are awarded in the following manners, at the sole discretion of the Organization:

Partial Grants are awarded toward any qualified applicant’s cosmetic dental treatment on a per capita, percentile basis, distributed proportionally based on the number of applicants in the distribution pool, the value of their recommended treatment plan, and the amount of proceeds available for distribution.

Eligibility

Program is open to all individuals that are ages 18 and over as of their application date. Applicants must be legal residents of the fifty United States of America. Applicants will only be given an orientation for distribution to a participating dental office within a 60 mile radius due to the nature of cosmetic dental procedures needing multiple appointments and proper follow up maintenance to ensure probability of success. Travel outside of the 60 mile radius to a dental office is strictly prohibited.

Verification of grant recipient and Delivery of grant proceeds: Grant recipient will be required to sign an affidavit of eligibility, a liability/claims release, and a publicity release. Awards may be forfeited if affidavit is not signed and returned within 30 days of notification to recipient or if awarded proceeds are not used within 3 months from the date of notification. If a grant is awarded to a minor, it will be awarded, at our sole discretion, in the name of the minor’s parent or legal guardian. Recipient agrees to permit their use of name and/or grant information for promotional purposes within any medium without additional compensation to the extent permitted by law. Any such use with respect to minors is subject to prior approval from the minor’s parent or legal guardian.

General

Participation is void where prohibited or restricted by law and subject to all applicable federal, state and local laws, rules, and regulations.

The program will be governed by the laws of the province of Ontario, Canada and any applicable U.S. Federal laws. Any and all legal actions or claims arising in connection with this program must be brought in the regional municipality of the city of Toronto, Ontario Canada.

The Oral Aesthetic Advocacy Group is not responsible for garbled, incomplete, misaddressed, postage-due, or misdirected applications, mail or communications, for errors, omissions, interruptions, deletions, defects, or delays in operations or transmission of information, in each case whether arising by way of technical or other failures or malfunctions of computer hardware, software, communications devices, or transmission lines or data corruption, theft, destruction, unauthorized access to or alteration of application materials, loss or otherwise.

Receipt of any grant award is conditioned upon compliance with any and all applicable federal, state and local laws, rules and regulations. The Oral Aesthetic Advocacy Group is not responsible for electronic communications which are undeliverable as a result of any form of active or passive filtering of any kind, or insufficient space in applicant’s e-mail account to receive e-mail messages. The Oral Aesthetic Advocacy Group disclaims any liability for damage to any computer system resulting from participation in, or accessing or downloading information in connection with, this program, and reserves the right, at its sole discretion, to modify, cancel, terminate or suspend the program should any virus, bug, technical failures, unauthorized human intervention or other causes beyond the Oral Aesthetic Advocacy Group’s control corrupt or affect the administration, security, fairness or proper conduct of the program. In the event of any such cancellation, termination or suspension, a notice will be posted and winners will be selected from among all eligible applicants received prior to such time.

The Oral Aesthetic Advocacy Group reserves the right, at its sole discretion, to disqualify any applicant from this program or any other offer conducted now or in the future by The Oral Aesthetic Advocacy Group or any of its affiliates if his or her fraud or misconduct affects the integrity of the program. By participating in this program, each applicant agrees to warrant that s/he is eligible to participate in this program. By accepting grant proceeds, recipient agrees to release the Oral Aesthetic Advocacy Group, its directors, employees, officers, and agents, including without limitation, its advertising and promotion agencies, from any and all liability, loss or damages arising from or in connection with the awarding, receipt, and/or use or misuse of grant proceeds.

Grant recipients will receive a discretionary grant toward their cosmetic treatment plan. No substitution, cash redemption, assignment, or transfer of proceeds is permitted. The Oral Aesthetic Advocacy Group is only responsible for award delivery; not responsible for award utility, quality, or otherwise. Taxes, if any, are the sole responsibility of the grant recipient.

Caution: Any attempt by an applicant to deliberately damage any website or undermine the legitimate operation of this program is a violation of criminal and civil laws. Should such an attempt be made, the Oral Aesthetic Advocacy Group reserves the right to seek damages from any such individual to the fullest extent permitted by law in any court of competent jurisdiction within Canada or the United States.
